Commuting mainly occurs within the municipality, as distances to neighbouring towns are longer than in the south. For those wanting a university hospital environment focused on advanced care for northern Sweden, Umeå is the obvious hub.

Vrinnevisjukhuset in Norrköping is the region's second major hospital after Linköping, with broad emergency and specialist care for northern Östergötland. Commuting to Linköping is common, taking about 25–40 minutes by train, meaning many combine housing and work between the cities. Private primary care is well developed in central Norrköping. The city is a reasonable alternative for those wanting hospital-adjacent work without moving to a university city.

Östersund is located in Region Jämtland Härjedalen. Commuting is mainly towards Östersund where the county's only hospital is located. Distances are large — much of primary care takes place via smaller health centres around the county.
